How can I make a payment?

Mail your payment to the address shown on your bill. Please remember to attach the stub from your bill when mailing in your payment.

What are your Billing hours of operation?

Patients can call between the hours of 8:00 AM to 5:00 PM CT Monday through Friday.

What if I do not have health insurance?

You are still responsible for payment of any services performed. However, you can contact our billing department to go over any payment plans, discounts, or programs available for uninsured individuals.

Are you In-Network with my Insurance?

Madison Core Laboratories works with all major Government and Commercial insurers to provide in-network coverage. To verify the participation with a specific policy please call your insurance member services or reach out to our billing department.

How often will I get a bill?

We send bills once a month until the balance has been satisfied.

Do you offer payment plans?

Yes, we do. Please contact our billing department directly to set up a payment plan and make the first payment. Once a payment plan is established, you may pay online, by mail, or by phone.

Do you offer discounts if you pay in full?

We can offer discounts in some circumstances but not in others due to regulations. Please reach out to the billing department so we can review your claim with you and see if you are eligible for a discount.

I received an Explanation of Benefits (“EOB”) from my insurance company.
What does this mean?

An EOB is a statement from your health insurance company to let you know that it has processed a claim for healthcare services on your behalf. The EOB is not an invoice or bill. It is simply an explanation of charges. After the physician ordered a lab test, Madison Core Labs performed the test, sent the results to the physician, and submitted a claim to the insurance company.

The EOB shows the following:
– A breakdown of the covered amount by the insurance company
– Potential patient co-pays and deductibles.

It is a statement of the medical services you received and details on how you and your plan will share costs. You will not use this to pay any outstanding bills. Do not make any payment to Madison Core Labs based on an EOB.

Why did I receive a bill?

Your provider has chosen to send your lab test to Madison Core Laboratories. If you receive a bill from Madison Core Laboratories and have questions, please do not call your provider but rather contact Madison Core Laboratories directly at (877) 755-0140

What is the bill for?

Madison Core Labs has a menu of panels available for your provider to order based on your physical examination, your symptoms, and the provider’s clinical assessment. Many infectious disease organisms (i.e., viruses, bacteria, parasites) may have similar symptoms that are difficult to differentiate without a diagnostic test. Madison Core Labs has proprietary technology TEM-PCR provides accurate and timely information to the physician.

Why did I receive a check for my diagnostic services?

Instead of paying Madison Core Laboratories directly, some insurance companies will send a check to the patient, making it the patient’s responsibility to pass on the payment. On the back of the check, please endorse with “Payable to Madison Core Laboratories” and mail the check as well as a copy of the EOB to Madison Core Laboratories, P.O Box 1565, Huntsville, AL 35807.

What can I do if my health insurance denies the services because of diagnosis codes?

Madison Core Laboratories obtains diagnosis information from the ordering physician’s office. If your insurance carrier denied your claim due to the diagnosis code, please contact your physician’s office. Once diagnosis codes have been updated, your physician should fax the updated codes to Madison Core Laboratories so your claim can be updated with the new information.

Testing Questions

Did I need this test?

Only your physician can explain why he/she ordered a Madison Core Laboratory. These panels are designed to be used in accordance with the type of illness or symptoms you may be experiencing. Your physician receives results in 24 to 48 hours, so that he/she may provide rapid and appropriate treatment to you.

Does Madison Core Labs do COVID-19 tests?

We run COVID-19 samples sent in from partnering clinics and nursing homes. We do not swab for COVID-19 as an outpatient service.

What do I need to have to get lab tests done that are ordered by my physician?

You will need a photo ID (this is a must. We will not process labs or perform venipuncture without it), your insurance card, and your signed physician’s order from the doctor’s office requesting the labs. Your doctor may also fax your lab orders to our office at (256) 850-3186.

Do I need to make an appointment to get outpatient blood lab work?

No, we are a walk-in laboratory. However, for the safety of our staff and patients during the COVID-19 epidemic, we ask that there be only 2 patients in the office at a time to maintain a 6 ft distance and that you wear a mask.

My child has orders for lab work, can they be done at MCL as an outpatient?

Yes, as long as your child has physician orders for lab work and the child is 14 years of age or older. Proof of age may be required.
